(Royalty Waiver Agreement)

I/We, (The Name of the Artist/Group/Act and Applicants Name that appears below) hereby grant Lizard Lounge Radio (herein refered to as LLR) a non-exclusive license and permission to use my/our production, name, likeness and performance of my/our original music work (or excerpts there from) for the purpose of producing Radio Webcast programs on LLR.

I/We hereby waive the right to any and all payment or compensation for airplay of my/our original music work on the LLR internet broadcast facility and the LizardLoungeRadio.com website, and grant LLR the right to make limited use of my/our original music work, image and/or voice on the LLR website and in any radio webcast produced by LLR.

I/We understand that by granting this Material Release Form, the waiver shall remain in force until cancelled by either party by giving 60 days notice in writing.

I/We agree to indemnify and grant immunity to LizardLoungeRadio.com from any liability arising from such use of my and/or our original music work.
